Home Health Care is intermittent and temporary. A patient may receive home health care services for different number of reasons. It is essential for a patient to be home bound as certified by a physician. A person is considered home bound when there is an underlying inability or if a person requires taxing effort to leave home.

 
 

Skilled Nursing Services

Our highly trained Registered Nurses will perform a comprehensive assessment and evaluation within 24-48 hours upon receiving the referral from your Physician. Within this evaluation, our RN’s will determine the need of services such as Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker and Home Health Aide services. Our RN’s provide quality nursing care in the convenience of your own home, in accordance with the patient’s plan of care and state and federal regulations. Our RN’s will assess physical condition; health promotion and disease prevention; coordination of services; teaching and training activities; and provision of direct nursing care.

Licensed Vocational Nurse works closely with the assigned RN, Clinical Manager and Director of Nursing. With enhanced communications and level of acuity, the team coordinates on a daily basis to trend events that leads to prevention of any exacerbation and hospitalization episode.

Our nurses provide the following services (not limited to) in the comfort of the patient's home:

LVAD (Left Ventricular Assistive Device)

                Transplants and IV infusions

                Milrinone Therapy

                TPN Therapy

Treatment and Injections (IM, SQ, ID)

Diet/Medications Management

Wound Care/Vac and Ostoomy Care

Urinary Care

PT/INR Lab Specimens/Venipuncture

               Diabetic Management/Teachings and Foot Care

                Family/Patient Health Teachings

 

Physical Therapy Services

Our goals for Physical therapy is centered to a unique and individualized treatment plan for each and every patient. Our licensed Physical therapists are dedicated in providing therapeutic intervention to achieve maximum level of rehabilitation. Our Physical Therapist are knowledgeable in performing Initial assessment or Start of Care. They paint the picture of the patient admitted. They provide exercise program and uses advanced equipment such as Ultrasound Therapy and TENS Unit for pain management if not contraindicated.

 Our physical therapist (PT) performs therapeutic exercises related to a specific loss of function and involves the utilization of adaptive equipment and aid in rehabilitation. The PT will assess the rehabilitation needs and potential of the patient including the development or implementation of a PT home exercise program. An appropriate and medically necessary PT program may offer the following benefits (but is not limited to):

1.       Increased flexibility

2.       Range of motion exercises

3.       Muscle strengthening

4.       Balance and Gait Training

5.       Safe Transfer Training and Body alignment and mechanics

6.       Prosthetic Training and Post-operative Rehabilitation

Occupational Therapy Services

Our goals for Occupational Therapy is geared to a unique and individualized treatment plan for each and every patient that qualifies for this service. Our licensed Occupational therapists are dedicated in providing therapeutic intervention to achieve maximum level of rehabilitation and assist patients in developing maximum self-care activities directed toward the return of a former function such as dressing, eating toileting and bathing. Our Occupational Therapist are knowledgeable in performing Initial evaluations in accordance to their scope of practice. They paint the picture of each and every patient admitted. They provide exercise program and uses advanced equipment such as Ultrasound Therapy and TENS Unit for pain management if not contraindicated.

 The occupational therapist may formulate activities that will restore the patient’s ability to perform Activities of Daily Living (ADL) and Independent Activities of Daily Living (IADL) such as adaptive and assistive devices like eating utensils and equipment, home exercise program, devising a perceptual motor training, activities that will develop and improve motor skills, energy conservation techniques, Breathing exercises, upper extremities coordination and strength, Functional mobility and muscle control.

Our Occupational Therapy Assistants works closely with the assigned Occupational Therapist, Clinical Manager and Director of Nursing to ensure proper patient care. Speech Therapy Services

Our speech therapist otherwise known as a speech language pathologist /audiologist is a qualified professional that assist, directs, supervises, evaluates and provides treatment for the deficits and difficulties noted during the initial assessment. The goal of a speech therapist is to provide the patient with skills necessary to communicate effectively in his or her environment.

The Speech Therapist works on learning techniques to improve memory, speech, communication, improve oropharyngeal coordination (swallowing). They also has specialized programs to assist patients with tracheostomy to improve communication with use of vocalization aids, helps with cognitive deficits, oral motor and speech impairment.

 

Medical Social Services

Our Medical social workers are true advocate for patient’s needs. They will help patients and their families assist with Short/Long term care planning, counseling, and coordination of community and financial assistance resources. They will assist in solving any financial or social problems that are interfering with the process of recovering from an illness, surgery or life- changing event. All of our medical social workers are trained in social work or a related field and have active and licensed by the State of Texas. MSW’s are trained to help in these fields (but are not limited to)

  • Assessment and Evaluation of the patient’s individual needs

  • Preparing advanced directives, such as a living will or healthcare power of attorney

  • Applying for federal or state assistance programs, such as Medicaid, Meals on wheels, food stamp, medication help, etc

  • Education regarding alternative living arrangements

  • Overcoming unsafe, neglectful or potentially violent home environments

  • Counseling for life-style changes

 

Certified Nursing Assistants Services

A Certified Nursing Assistant is available to address the individual needs of the client, whether he or she is living at home or in another residential facility. They may aid in personal care and bathing. Care may include but is not limited to: Bathing and Dressing, Assistance with ADLs and Personal Hygiene Care
