WHO CAN RECEIVE HOME HEALTH?

Home Health Care is intermittent and temporary. A patient may receive home health care services for different number of reasons. It is essential for a patient to be home bound as certified by a physician. A person is considered home bound when there is an underlying inability or if a person requires a considerable and taxing effort to leave home. A patient, may leave home, but it has to be in short duration and not often. Examples may be, trip to pharmacy, doctor’s appointment, religion service, etc.

A medical necessity is also required. Medical necessity is the primary reason why home health is needed. Examples may be but not limited to: recent hospitalization, wound care, IV, recent falls, new diagnosis, infection, medication management, poor endurance and unstable gait, etc.

We at Home care of Texas, will serve as your doctor’s eyes and ears. We will paint a picture of your health condition at home and will coordinate that with your physician. It is important to know and realized that, in a hospital setting, patient’s are in a controlled environment. Once a patient is discharge to home, there are so many factors that will affect the care. It could start from medication management all the way to home safety assessment. We will be there not to only to assist with the transition but to serve as an educator to prevent further hospitalization. We will report, intervene, execute, coordinate, trend results and most importantly advocate for our patients to make sure they receive the best quality care and achieve their maximum level of independence. We will ensure that we use evidenced based practice to meet our goals.

What is your business, and who are your patients?

Home Care of Texas is a licensed and certified home health agency that services Houston/Katy and its surrounding cities.

Our patients are mostly geriatrics but not limited to, that had some sort of exacerbation and is needing a full recovery in the convenience of their own home.

Where do you get your patients?

Home Care of Texas is a preferred home health provider by major hospitals in the Houston area. We get most of our referrals from the hospitals, specialty physicians, doctor’s offices, in-patient rehabs, nursing homes, and other facilities that needed a dependable home health agency. We also receive self-referrals.

What are the limitations of Home Health?

Home health is a temporary service that needs certification from physicians. A face-to-face encounter must happen 90 days before admission or 30 days after admission to qualify for home health care. Home Care of Texas does not provide everyday visit to patients. Since home health is a temporary benefit, Home Care of Texas’ priority is to teach the patient and the family ways to navigate independently to the new situation sustained relating to the most recent exacerbation.

What documents are needed to start home health?

First, an actual face-to-face encounter from physician dated 90 days before admission or 30 days after admission. Second, a signed physician or a qualified practitioner’s order for home health and lastly, a signed copy of the actual face-to-face encounter.

I want someone to stay with me for hours at home, is this possible?

We get this question all the time. Unfortunately, this is called a provider service through Medicaid, VA or insurance benefit. Home Care of Texas can help and navigate you to this goal by sending you a social worker to help with provider services qualifications and community resources planning.

What types of insurances do you accept?

Most of our patients have traditional Medicare, however, depending on your out of network benefits, we may be able to help.  We have patients under BCBS, United Healthcare and Workers Comp at the moment. We also accept private pay.

I do not have a primary care doctor, but I need help, can I still get the home health benefit?

Yes! Home Care of Texas is affiliated with qualified practitioners who makes home visits.

What is your biggest accomplishment as a home health company?

Home Care of Texas’ biggest accomplishment to date is the ability to bring in an array of professional individuals who are experienced in their own filed to help and change one patient/family at a time. This interdisciplinary approach that centers to the use of evidenced based practices is what separate our home health from the rest.

Scheduling and Compliance

The beauty of home health care is that you and your clinicians control the time and day of the visit. The ability to navigate and compromise with the schedule is a great way to gain trust and compliance. Different patients with different social determinants of health would require different approach. Our staff are trained to determine social determinants of health and find the balance on the abilities and capabilities of each and every patient.
